These forms are for churches that have a parsonage. Please
note each year one parsonage review form and one parsonage inventory form needs to be sent to the district office.
Parsonage Review (click to download) - fill out either this form by downloading it, printing it out, and then mailing
or faxing it in to our office. Please note signatures needed to be on the form prior to your faxing it back in to our office!)
-OR- Parsonage Review (form fill in) - to use this form, click on
it and it will open in Word. You can type on the form right from your computer by using your "tab" key to move from
field to field. Save it in Word to your hard drive. Please name it so it is easy for your office to recognize (i.e.,
"MYCHURCH_parsonage_review_2010.doc). Print out and mail or fax the copy to our office. Please note signatures
needed to be on the form prior to your faxing it back in to our office.
Parsonage Inventory - please complete and fax this back to the district office with the
parsonage review form.
